Copy of itinerary OR round trip airline tickets OR a Business letter addressed to the Passport Agency on an official letterhead stating the exact dates of travel and purpose of the trip OPTION

Visit your local Post Office or Clerk of Court to get all the documents listed below executed and sealed (Appointment May Be Needed) – Make sure to tell them that you want to do a hand- carry. DO NOT OPEN THE SEALED ENVELOPE - You will need to send us the UNOPENED envelope with the documents. Local Acceptance Facility

Present Proof of U.S. Citizenship -US Original or certified Copy of Birth Certificate, or Certificate of Citizenship or ORIGINAL Naturalization from BCIS.
